Gangnam Veneers: A Treatment Case for Gapped Teeth with Gum Inflammation

By Park Jonguk, DDS, MS — board-certified specialist in conservative dentistry Published

Hello, I'm Park Jonguk, director of Dream Dental Clinic.

Gangnam veneer treatment is done for a wide variety of reasons and purposes.

Most of the time, the goal is simply to make the teeth look nicer.

But occasionally, veneers are done for a purpose other than this aesthetic one.

In this case, the gums were severely inflamed, and through veneer treatment

we set out to clear up the inflammation

while also making the teeth beautiful — let's look at that process.

Generally, when veneers are placed,

gum inflammation that wasn't there before can sometimes develop.

Because of the veneer ceramic,

the gums can become irritated,

or inflammation can develop from poor brushing,

or inflammation can arise from a buildup of tartar around the veneer.

For veneer patients who come to our clinic,

I spend quite a long time going through various photos and explaining things,

and I always make sure to talk about the possible side effects.

One of the side effects of veneers

is that gum inflammation can easily develop if brushing isn't done well.

This case, on the other hand,

is a process of clearing up inflammation through veneer treatment.

You can see severe gum inflammation — that is, gingivitis.

The cause of this gingivitis was heavy tartar

and a poorly done resin treatment.

As the teeth developed a gap,

space had formed between them,

and that space had been treated with resin.

The resin treatment was done, but the teeth had gapped even further again.

The problem is that filling the space between teeth with resin

makes it easy for inflammation to develop underneath

if the treatment isn't done quite well.

On top of that, without regular scaling,

tartar had also built up, making the gingivitis much worse.

Through Gangnam veneer treatment,

we needed to make the teeth look nice and bright,

close the space between the teeth,

and clear up the inflammation — a difficult and complicated task.

First, we perform scaling and remove the resin attached to the teeth.

Then we wait for one week.

You can see the gum inflammation has cleared up dramatically.

When there's space between the teeth,

no-prep veneers are possible,

but in a case like this, where brushing hasn't been effective

and gum care hasn't been good either,

doing it without any reduction makes it very likely that inflammation will return.

Through a moderate amount of reduction,

the Gangnam veneer needs to be made with no gap against the tooth,

and it must meet the tooth surface very smoothly —

only then will bacteria not accumulate and tartar not stick.

Through a precise diagnosis,

we reduce the tooth by exactly the amount needed.

The reduction is done in a way that allows the veneer to be made without irritating the gum.

Of course, precise Gangnam veneer fabrication skill matters too.

With a same-day method, in a case like this,

it's very difficult to produce a result that reduces the inflammation.

The handmade method delivers better results.

Gum contouring is carried out at the same time, along with making temporary teeth.

After making the temporary teeth for the upper teeth like this, treatment begins on the lower teeth.

For the lower teeth as well,

so that the veneer doesn't irritate the gum,

we reduce only as much as needed to make fabrication possible.

Temporary teeth are made for both the upper and lower arches,

and we wait for the gums to stabilize during the temporary-teeth period.

This is the state of the gums two weeks later.

The inflammation has completely cleared up, and the gums have returned to a healthy, firm state.

With the gums stabilized like this,

we precisely fabricate the veneer using ceramic.

If the inflammation doesn't clear up during the temporary-teeth period,

we keep adjusting the shape of the temporary teeth

and wait until the gums stabilize —

only then will inflammation not return after the veneers are placed.

This is the appearance after the veneers were set.

You can see that the gums are all very healthy and firm.

So that the veneer doesn't irritate the gum,

it's been designed with great precision.

All treatment has been completed.

The teeth are bright and beautiful,

and the gums are very healthy —

now it's just a matter of good maintenance.

Let's look at the appearance two years later.

Compared to the photo from two years ago,

the gums have actually gotten even better.

The teeth remain just as bright and beautiful as well.

With a precise plan like this,

we decide on a method to clear up the gum inflammation,

and finish with a veneer that has been finely crafted to match it —

only by doing it this way

can the inflammation be cleared up,

the space between the teeth closed,

and the teeth made beautiful while the gums stay healthy, all at once.
