Cheongdam Veneers: A Treatment Case for Protruding, Stained Teeth

The case that requires the most caution when doing veneer treatment

is veneers on protruding teeth.

In fact, to be strictly honest, veneers for protruding teeth

are not easy to recommend.

In many cases, a large amount of the teeth has to be cut down, which ends up harming the health of the teeth.

Depending on the case, it's important to carefully screen for which cases are actually feasible.

When the protruding mouth is due to a skeletal problem — in other words, when the bone itself protrudes —

trying to resolve it with veneers is often meaningless.

When the teeth are sticking out,

if the entire tooth is sticking out, the amount of reduction needed becomes large.

When only the tips of the teeth protrude, there can be some meaningful effect,

and that's the case we'll look at here.

Before even getting to the protrusion issue, the teeth had too much staining,

and there was also a problem with the shape of the teeth itself.

The teeth looked overly full, and their shape was also too round,

making it hard for the smile to look pretty when smiling.

Viewed from the side, you can see that the angle of the teeth is quite protruded.

Even though orthodontic treatment had been done, various issues remained unresolved.

Since it was a case that couldn't be resolved even with re-orthodontics,

we went with Cheongdam veneers to resolve

the tooth shape and staining together with the protrusion.

There are two solutions available here.

One is to reduce a large amount of the teeth and bring the teeth inward overall,

but doing that causes problems for tooth health,

with worsening detachment and fracture, and it can also cause tooth sensitivity.

We decided to treat it by only changing the angle of the teeth, without a large amount of reduction.

We mark which areas need reduction,

then grind down that area on the plaster model,

and place wax on top of it to test out in advance what shape the treatment should take.

Through this wax-up process,

the amount of tooth reduction and the expected outcome can be confirmed before treatment.

We check this wax shape together with the patient like this,

and then move into treatment.

Tooth reduction is carried out according to the plan.

Since this was a case where the teeth needed to come inward,

the amount of reduction is a little more than with typical Cheongdam veneers.

The entire reduction process is carried out using precise guides and a microscope.

The teeth ended up looking even and clean,

while also finishing beautifully and naturally.

Even with extremely thin ceramic,

the natural three-dimensional feel and shape of the teeth,

as well as blocking out the tooth color to give a bright feel,

and even the translucency seen at the tips of young, healthy teeth,

were all expressed.

So how was the protrusion resolved here?

The way to effectively resolve it while minimizing reduction

is to make the gum side a bit thicker,

while bringing the tip of the tooth inward,

which is best done in a way that creates a sense of the tooth rotating.

Giving this sense of rotation dramatically reduces the feeling of the teeth protruding.
